- Assisting clients and whānau with accessing and navigating health and wellness services within communities
- Ensuring the delivery of effective healthcare and support services through delivery of tasks including:
- Performing clinical tasks as directed by healthcare professionals (after appropriate training and/or certification)
- Facilitating the delivery of Koiora services, including the Diabetic Retinal Screening (DRS) service
- Supporting an effective reception service at Koiora
- Support for enrolment into general practice services
- Empowering clients and their whānau to improve their self-management of health and social challenges by ensuring that services are undertaken in a person centred, individualised manner that upholds the values of the RAPHS

Description
RAPHS purpose is to enable high quality health and wellbeing services for our community, with a focus on supporting the delivery of health care through an integrated network of care providers.
RAPHS delivers services for the enrolled population and those people who have complex medical or social needs to provide case management/support services for a period of time following hospital discharge.
The Paeārahi – Health and Wellbeing Navigator plays a pivotal role in providing a support system to people who are engaged in RAPHS services and providing support and navigation to help meet their needs.
This role will work in partnership with RAPHS clinical teams and other providers, with responsibility for:
