    Responsibilities

    • Develop and implement customized care plans in accordance with each patient's unique needs.
    • Perform home visits, administering medication, wound care, treatments, and procedures as required.
    • Monitor clients' conditions, reporting changes to clinical or client services managers.
    • Document observations, interventions, and evaluations using state-of-the-art touch pad tablets.

    Requirements

    • CURRENT LICENSE AS A REGISTERED NURSE IN NEW HAMPSHIRE.
    • MINIMUM OF ONE YEAR OF RECENT, VERIFIABLE CLINICAL (MEDICAL/SURGICAL) EXPERIENCE.
    • PRIOR HOME CARE EXPERIENCE STRONGLY PREFERRED BUT NOT REQUIRED.
